Do we have to use FaxRecall for Claim information
or to verify benefits?
You can receive verbal confirmation regarding a
payment to your provider, or your provider can receive written
confirmation regarding payments to them, your benefits, your deductible,
and if you have reached your deductible. This is a fast and fun way to
verify your information. Please go to our FaxRecall tab and see how easy
it is to quickly receive this information.
Is
my doctor in the Network?
We have included most of the provider links for you
to check, but the safest way
is to call your physician or facility directly and ask them. There is also a phone number for your insurance card that you
may call.
What
is the mailing address for sending Claims?
This information is located on your insurance card. Claims may also be submitted electronically by your provider.

Where do I call if I need it?
The information is on the back of your insurance
card. Most of the time the
facility will make the Pre-certification request, but it is the
responsibility of the insured to make sure they are pre-certified.

maternity covered under my plan?
See summary plan document, or your certificate
label located on the inside cover of your booklet.
What
is your fax number to fax in claims?
You may fax in your claim to
405-751-8076 if it is a non-network claim. Your provider can also submit your claim electronically.
What
will the FaxRecall send me?
Look at our FaxRecall tab and see the format of
information our FaxRecall system will send to your provider.
What
is patient’s responsibility?
This information is found quickly using our
FaxRecall system, or see the Explanation of Benefit (EOB) sent to you
regarding the claim.
Has
my deductible been met?
The EOB will show all charges being applied to your
deductible.
Who
is the claims supervisor?
You may leave an e-mail
message for a supervisor at claimsmgr@epstpa.com.
Please include your name, social
security number, and group number or name.
Your questions will be answered quickly and efficiently.
